Learning from incidents for improving safety within dangerous operations
Learning from accidents and near-misses (below, commonly named incidents) is one of the most important ways we know in order to avoid repetition of serious events and to improve long-term safety within an organisation and its operation. To be able to improve the learning process it is of great importance to map and to understand how the learning is accomplished in various organisations.
The purpose of the project is to examine the learning processes around incidents in general and the connections to the safety culture of the organisation in particular. We want to know what aspects of the safety culture that support the learning and what aspects that can constitute obstacles.
